Health Care Center - OPEN TO THE PUBLIC!

SKILLED NURSING The Freedom Village Health Center provides skilled nursing, rehabilitation and long-term care on a 24 hour basis. Rooms are semi-private with a shower in each room. Services are for people who: Are recovering after an acute illness, surgery, or hospital stay Are in need of rehabilitation after a stroke, fractured hip or other decline in independence level Require long-term, supportive care and moderate or extensive assistance with activities of daily living such as dressing, bathing, walking, transferring, eating and bathroom use Need compassionate, supportive care in the final stages of a terminal illness Require temporary or permanent nursing assistance Suffer from Alzheimer's Disease or related disorders

Directions: Exit the 405 or 5 freeway on El Toro Road. Turn east and drive for approximately 1 mile. Turn right on Cornelius (just past Raymond). The Health Care Center is at the end of the cul-de-sac. Take the last driveway on the left to reach the entrance.
