Directions: Exit the 405 or 5 freeway on El Toro Road. Turn east and drive for approximately 1 mile. Turn right on Cornelius (just past Raymond). The Health Care Center is at the end of the cul-de-sac. Take the last driveway on the left to reach the entrance.
